Permanent Records Transfer
Use these forms to transfer permanent records to the State Archives

Records Transmittal Form (ARM-14) - used to transfer permanent paper records to the State Archives via the State Records Center staff

Digitized Records
Authorization for Disposal of Original Paper Records Stored as Digital Images (ARM-13) - used to destroy original paper records now digitized and to designate the electronic copy as the official version of the record

Microfilm
ARM-8A Microfilm Transmittal and Receipt (ARM-8A) - used to transfer security microfilm to the Archives' vault
ARM-12 Microfilm Quality Certification for Records Disposition (ARM-12) - used to dispose of original paper records now microfilmed
Microfilm Certificates and Targets - meet the Department of Archives and History's standards for microfilming
